To start, we'll need your organization name, mailing information and tax identification number (TIN).
To assign multiple users to your Optum Pay account, please provide the contact information for each designated Optum Pay admin.
In order to get funds into your preferred account, please include your routing and bank account information.
Lastly, have a signed and dated copy of your organization’s federal W-9, along with a voided check or bank letter for each account where payments will be deposited.
